Sample Email to Instructor

My name is ___________ and I am registered with the Student Disability Center (SDC). You recently received a Letter of Accommodation from the SDC listing the academic accommodations that I need.

I would like to set an appointment to talk to you to make specific arrangements for the accommodations   relating to testing or classroom needs,  and how they will be provided. I appreciate your time.

My best times to meet are [give a couple blocks of time on 2 or 3 different days]. Please let me know if you are available during any of these times so we can schedule a meeting.

If you prefer, we can try to work out some or all of the arrangements by email. Whether or not we meet, we will need to have a written agreement (on paper or by email) specifying the details of the accommodations such as the date, times, and locations of exams with extended time, the location of a separate testing room, etc.
